We are located at 200 Theda Clark Medical Plaza located on the Theda Clark Medical Center Campus. Free valet parking is available at the main entrance. Enter at the main entrance of the hospital and turn right into the Physician Office Building. Take the elevator to the second floor. We are to the right in Suite 270.
