At Women’s Health Specialists, we are proud to provide our patients with the best services available for your health care. As a courtesy, we will submit your claims to your insurance carrier; however, you are ultimately responsible for the payment of all services in a timely manner. If you do not have insurance coverage, we require you to set up an appointment with our financial counselor, Gloria, in order to discuss your payment options prior to your first appointment. Please be advised that you might be required to have your laboratory or ultrasound services provided at either Appleton Medical Center or Theda Clark Medical Center’s outpatient services. If you have any questions, please feel free to contact our financial counselor at (920) 749-4007 Ext 153 or by email Financial.Counselor@foxvalleyobgyn.com. We look forward to hearing from you.
Co-Payments: Any required co-pays or charges not covered by the patient's insurance plan must be paid at the time of service. For your convenience Women's Health Specialists accepts cash or checks, as well as all major credit and debit cards.
If you have BadgerCare+ / Medical Assistance, please note we are United Health Care MA providers. We do not provide services for Network or Managed Health Services for Medical Assistance even if you have other coverage. Your BadgerCare+ must be verified as active prior to all appointments.
If you are a student/dependent with coverage under a parent’s insurance plan, please be advised that even if the payment goes to the subscriber, you are responsible for the payment of all services provided. If you have any questions, please speak to our Finance Office and we will be glad to assist you.
Failure to remit timely payments could affect future services in our clinics. Accounts are due, in full, upon receipt of your statement. If you are unable to pay your account in full, please call and speak to our Financial Counselor to discuss your payment options. (920) 749-4007, x 153
If you have secondary insurance coverage or medical assistance, please provide this information at your first visit, or when the coverage becomes active. If your insurance coverage changes or is terminated, it is your responsibility to contact our office immediately to provide that information.
Patients who have insurance coverage that pays the patient directly are advised that such payment must be remitted to our office, upon receipt, along with a copy of your EOB (Explanation of Benefits) in order to receive any discounts associated with your insurance provider. You will be responsible for the full charged amount until the EOB and payment is received.
