Register as a New Patient

Welcome!

We'd like to take this opportunity to welcome you to our practice and to thank you for choosing our practice. We are constantly striving to make our patients' visits more efficient. We will send you a welcome letter with your appointment details along with our patient Information form. Please complete the patient Information form before your appointment and bring It with you to your appointment. If you have any questions feel free to contact our office at (920) 749-4000. Prepare for your Office Visit

In addition to completing the above forms, we will also require the following information for your first visit:

  • Driver's License or Other Valid Government-Issued Photo Id
  • Insurance card and any co-payment due for visit
  • We share an electronic medical record with the Thedacare system. We are able to access other offices notes and labs or Imaging studies that you have recently completed.

If you are coming to us from another healthcare system It would be helpful if you requested a copy of your recent medical record be sent to us. Please list the physician you are going to see on the form so that your records get to the right location.

Please arrive 15 minutes early if you have not yet filled out these forms. If you have completed forms with you, you only need to check in with the reception desk 5 minutes before your scheduled appointment.
