Register as a New Patient
Welcome!
Welcome to our practice and thank you for choosing Women's Health Specialists. We strive to make our patients' visits efficient. Call us today to register as a new patient. We will ask you for basic information over the phone; then we will send you a welcome letter with your appointment details along with a patient information form. Please review the patient information form before your appointment and bring It with you. If you have any questions feel free to contact our office at (920) 749-4000. We will be happy to answer your questions and assist you.
Prepare for your Office Visit
Bring the following for your first visit:
- Insurance card(s)
- Photo ID (Driver's License or State Photo ID or Passport) - Required by the Federal Trade Commission (FTC) to verify the identity of all new patients
- Co-payment due for visit (We accept cash, check, credit and debit)
- Patient Information Form
We share an electronic medical record with the ThedaCare system. We are able to access other office notes, labs, or imaging studies that you have recently completed. If you are coming to us from another healthcare system It would be helpful if you requested a copy of your recent medical record be sent to us. Please list the physician you are going to see on the form so that your records get to the right location.
