Pay Your Bill

Please feel free to ask us about our fees. We will be happy to discuss them with you. Our fees are within the customary range for this area and reflect the level of care you will receive and the high level of training by the physicians in the group.

Please note that hospital charges are always billed separately and are not included in physicians' fees.

Patients are billed monthly. If there is an error or question on your account, please contact our billing department. Many times a phone call can help clear up any misunderstanding about your charges.

Please bring your insurance card or your Forward card with you at the time of each visit. As a convenience to you, our office will submit your charges for services to your insurance provider. However, you are responsible for any charges not covered by insurance.

For your convenience we accept cash, checks, Mastercard and VIsa. You may make a payment by phone, mail, or in person at our Billing Services Office.
