Pay Your Bill

Please feel free to ask us about our fees. We will be happy to discuss them with you. Our fees are within the customary range for this area and reflect the level of care you will receive and the high level of training by the physicians in the group.

Please note that hospital charges are always billed separately and are not included in physicians' fees.

Patients are billed monthly. If there is an error or question on your account, please contact our billing department. Many times a phone call can help clear up any misunderstanding about your charges.

Please bring your insurance card or your ForwardHealth card with you at the time of each visit. As a convenience to you, our office will submit your charges for services to your insurance provider. However, you are responsible for any charges not covered by insurance.

 You can pay your bill over the phone with your credit, debit or Flex Spending Account card.  To pay by phone or discuss payment options, call 920-749-4000 and ask for the Billing Office. You may also make a payment by mail or in person at our Billing Services Office.

  • Current billed charges for these services
  • The average allowable payment from Medicare for these services
  • The typical charge in this area for these services according to Wisconsin Health Information Organization (WHIO) Datamart Version 4 (DMV4) database in cooperation with the Wisconsin Medical Society
